Here, you will find all necessary forms to co-manage and bill Medicare for your participation in cataract surgery, to complete the co-management requirement for refractive surgery, and those required for glaucoma certification. We strongly believe that co-management can provide the best all-around care for patients, and that co-management success is dependent upon fluid and detailed communication between your office and ours.
